Francesca Rinaldo ’13, ’15

Francesca Rinaldo, MD, PhD, is the director of value-based care for Kaiser Permanente Nevada, where she leads strategic initiatives to improve quality, access and affordability through innovative care delivery and population health programs. She partners with clinical and operational leaders to strengthen interdisciplinary care, improve patient outcomes and advance value-based health care across northern Nevada.

Since joining Hometown Health in 2025, Dr. Rinaldo has led the development of key initiatives, including the Del Monte Clinic, the Comprehensive Health Assessment Program and care management programs serving members with complex medical and social needs. Her work emphasizes care coordination, operational excellence and data-driven performance improvement.

Dr. Rinaldo also served as chief clinical innovation officer and corporate medical director at SCAN Health Plan, where she led population health and complex care programs for Medicare Advantage members, including oversight of the organization's Institutional Special Needs Plan (I-SNP). She previously served as senior vice president of Clinical Product and Innovation and chief medical officer at CareLinx by Sharecare, leading the national expansion of technology-enabled home-based care programs.

Dr. Rinaldo's career spans academic medicine, digital health, health plan leadership and value-based care transformation. She completed clinical training in general surgery at Stanford University and a fellowship through Stanford's Clinical Excellence Research Center (CERC), experiences that shaped her passion for designing innovative models of care that improve quality, patient experience and health care value.

She earned her bachelor's degree from the University of Chicago and her MD and PhD from the University of Louisville School of Medicine.
